What to Bring

When you come for your appointment, please remember to bring:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Any relevant reports, lab results, and any other important information
  • List of medications (if any)

New patient paperwork & online check-in

If you provided your email address and cellphone number, you may be eligible for online check-in. If eligible, you will receive an email or text notification. Check-in may only be completed through Modernizing Medicine’s APPatient app. If you are not eligible for online check-in, you may choose to print and fill out our new patient packet below and bring it with you to the appointment.

How to cancel an appointment

To cancel your appointment, please telephone the office during business hours and allow at least 1 day’s notice so that we can offer your appointment time to patients on our waiting list. We recognize that your time is valuable and make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
